The tooth is three layered Enamel (Outermost), Dentin (Middle) & Pulp (Innermost). In the event of a poor oral hygiene the bacteria sitting on the surface of the tooth digs deeper all the way into the Pulp destroying and causing infection. In such cases root canal treatment will be performed where the pulp is cleared of the infections and a crown is placed after a few days. Unless it affects only the second or the first layer of the tooth, a basic dental filling would suffice.
